Practice, practice, practice. I know when I'm in a new situation, it takes me a while to get comfortable. For example -- shooting in a new gym or playing with new teammates. Usually, once I get a feel for the game I'm fine. The more you play with your potential teammates, the better off you'll be.

Everyone can have bad days offensively but a good defender doesn't have to score at all to have a good game. When your trying out guard the best player out there and shut him down. No matter how bad you are playing offensively you can always play good defense and rebound. The things coaches hate the most are turnovers but if you do turn it over then make up for it on the defensive end, coaches love that.

You can cover up some of your mistakes with extra effort and hustle.
So you just let someone blow by you on defense? Instead of giving up, chase him down, shout for help, just don't quit on the play.
Blew a wide open shot? Well don't just stand there sulking! Chase down the rebound!
Someone just blocked your shot? Don't stand there looking like an idiot, keep your eye on the ball so you can get it back.
Anyways, the point is that even if you make a mistake, don't have a loser's attitude and just give up. If you coach isn't an idiot, he'll appreciate the extra effort you're putting out and take that into consideration.

First of all I gotta commend you for trying out ALL four years despite not making it the first three. That's pretty damn cool, kid.
To work on your consistency, practice. It's as simple as that. Play against good competition to hone your skills and work on your confidence.
To work on your nerves...hell, this is your 4th crack at this. If you're not used to the pressure of the tryout process yet then I dunno what to tell you.
What you need to do is play within yourself and your set of skills. You say you know when to take a good shot, so take it. Play your game. If you try to play the way the coaches want you'll never make it if that's not your style. If you do somehow make it, you won't be happy.

you just gotta take more shots. take 10 shots each spot. try to get a higher percentage each time. Work on your ball handling with different drills so you don't lose the ball. Practice using your ball handling moves and accelerate through them. Consistency comes from repetition.
