Yes is harmful and could be a bad gasket on the manifold or if your car has gasket at the end of the manifold that joints the exhaust or just a bad exhaust. Also, are cranking the car in a garage? Check if your outside vent for the AC is open and the wind is blowing the fumes towards the car. Check for a faulty EGR valve, might be leaking in the engine compartment and the fumes are coming through the AC vent.
Whatever is, get it check. At least you can smell it, but CO is odorless, colorless and if you feel sleepy you are getting poisoned with it.

Either you have an air-cooled engine (older VW) with a bad heat exchanger, or a bad exhaust system in a car that has an unsealed passenger compartment.
Make sure that you keep at least one window cracked. The exhaust leak may be a bad gasket, cracked header or a rusted out pipe.
There is the chance that what you think is an exhaust smell is really a antifreeze. That means that the heater core is shot. On some cars this is an easy repair. But on others it requires that the A/C unit also come apart to get it out. In which case take it to a pro that specializes either in your make and model (not always the dealer) or the system (A/C).

People commit suicide by routing the exhaust into their car. It's probably the exhaust manifold if it's coming through the vents.

The vent air intakes are not even located close to the exhaust on any passenger car....therefore your concern would require some investigating to assure that you don't harm yourself with the window rolled up...

it is obviously an exhaust leak. bad gaskets, bad muffler, or a bad manifold "do-nut." i had a problem with a classic car i drive and it caused a back ache. i didn't connect the two until i went to the hospital because of the pain. after several blood tests it was determined that i was suffering from carbon monoxide poisoning. my kidneys were trying to filter it out and they were being over-worked. this caused the pain. please be very carefull!!!

It is exhaust gases. Yes it is very harmful it can kill you. You probably have a leak off of the header.
