Mat 12:31 Wherefore I say unto you, All manner of sin and blasphemy shall be forgiven unto men: but the blasphemy [against] the [Holy] Ghost shall not be forgiven unto men.
There have been volumes written about what this could be. Now, you will get my take.
Let's presume a person is saved, to the point that they are "on fire" going and preaching the gospel and getting others saved too.
Then one day, the person wakes up and suddenly decides that he was wrong. That although there may be a God, it certainly isn't limited to Jehovah. And even though Jesus was a good man, He wasn't the sin offering of the world.
He further decides that he will deny that Jesus did not die for him and he renounces his salvation.
Now, flash forward to the end of the man's life and on his deathbed, he dies without recounting his denial of Jesus.
This is the blasphemy of the Holy Spirit - Not only did he know salvation's grace and accept it, but he pushed it away and died without it.
There are some denominations that teach "once saved, always saved". When analyzed in the above model, and context of the scripture, we find that teaching is in error.

Here is another example of Blasphemy of the Holy Spirit:
A young man hears the gospel and decides that it is not for him. He goes on to die without ever accepting Jesus as Savior.
And another: A young man is born in the middle east and follows the teaching of Islam to the letter. Leading a righteous and relatively sinless life, he too never accepts Jesus as the sin offering of the world.

John 3:16 doesn't have a section that starts with except! There are no parts that say unless your a bad person.
