I think this Form 4605 is not an IRS form 4605. I looked up Form 4605 on the irs website, and it says that it is this
Form 4605, Examination Changes—Partnerships, Fiduciaries, Small Business Corporations, and Domestic International Sales Corporations—used to report audit changes made to partnership, fiduciary, and small business corporation returns
What form are you really looking for?

I think that you have something wrong. The IRS does not do "Refund Anticipation Loans". Only the tax prep places do.

Do you mean a refund anticipation loan form? If it is, that is a form or a contract paper between you and Tax preparers who offer refund anticipation loan, such as H&R Block. And I do not think there is an IRS form 4605.
