"Hi, I'm Fred". Unless your name isn't Fred, of course. Then that would be confusing, as well as embarrassing.
Honestly - just about anything that shows you have something in common. If you're in a class with her, ask if you can ask her a question about a homework assignment. If you don't live nearby but she does, ask for directions to a store. If you both have a common friend, ask about the friend.
Also, ask open-ended questions that need more than a "yes/no" answer. Ask her what she thinks of the assignment, or if she would recommend another store than the one you picked. This gives her a chance to talk a bit more than 3 words.
After that, you're on your own.

I guess it depends on the situation and where you are. Saying something nice about something she is wearing I think is always good. Or tell her you think she has beautiful eyes or a smile.

it depend if you been around her before...(will it also depends how old r you).. but i guess hi! how are you?? and let her konw you dont wanna freak her out ... that you just wanna talk to her ... if she ok with it ... but once again does she even know that you exist?? that makes things a lil bit better .. good luck =)
