To replace a green card (aka a Permanent Resident Card) due to an error caused by the INS, you will need to submit an I-90. The instructions, fees and everything else to submit one can be found in the instructions section of the form which you can get here:

As long as the date of birth is correct, you should be fine. If the age is one year, then its possible that there is a glitch in the year.
edit: what card do you have? I've just realized that there is no age on my permanent resident card.

Careful if it isn't your card, especially if you found it and the owner reported it missing etc. If you don't have the proper papers to prove who you are other then the green card you are asking for trouble, then I'd leave it be unless you want to say bye to America even if it really is yours. Unfortunately you would be guilty of fraud until proved otherwise. Happened to a friend who was nearly deported ;)
