Yes, actually twice, once when entering the exam room and again after examining the patient. I did this for two reasons. First, I believe in the germ theory of disease. The second reason is less altruistic, but I thought that many parents would be turned off if I did not wash. As I got older, the natural aging process took its toll. In wintertime especially, my hands would be very chapped. They would be actually painful when I went to bed at night. Now retired, my hands feel great.

The women physicians in the office were more fastidious about using hand cream repeatedly during the day. I guess I should have learned from them.

I'm sure I forget every now and then, but as a general rule, yes.
I wish I could cite the source, but a few years ago, somebody had researchers hanging out in the restrooms at a convention, surreptitiously taking a tally. Something like 60% of the men, and I think about 40% of the women didn't wash their hands after going to the bathroom. The convention? Infectious disease specialists!

Yes it is manditory and that goes for all of the staff as well. The hospital I work at has a protocall that you need to wash you hands where the patient can see you doing it before and after examination or check up.

major consequences happen if patient reports concern of unsanitary environment including hands.
