When my son was born in US at city of San Gabriel in Los Angeles, California & already to made his birth certificate with his chinese give name. but i call his nickname at home so he know his nickname more than his givename therefore when he is going to school is hard for him to call. If he uses his nickname at school need to make the birht certificate with givename & nickname together. But i don't know how to fill out his certificate, because i didn't see it has nickname to fill in. please help me

Go to your local county office & ask them what to do.

You don't put a nickname on a birth certificate, that's part of what makes it a nickname. When he enters school you can just tell the teacher "His given name is _____, but everyone calls him _______ so that's what he's most comfortable with." If you wanted to change his legal name then you could do it by filling out some forms, but that's not really necessary for him just to be called something else by his peers.

Nicknames do not go on birth certificates - that is for full legal names only.

When teachers call roll the first day of school, they always ask if they are called by something else. If they don't, then your son can just say "I go by _____"
