Generally it is because:--

A) When covering a major story that requires material to be sent back to the newspaper office/TV station/Wire service etc, the journalist asks people in charge of the scene for details, or eye witnesses, and often these details are incorrect. It is only after some sense of order is established that officials have more accurate facts and then pass these onto the newsmen.

B) Some journalists (a very small group) are lazy in doing their job. I have known war correspondents working for wire services (Reuters and UPI) that did not go into the field and see and collect information themselves, but relied upon government handouts or speculation from "Diplomatic sources" etc.

Ratings, ratings, ratings, ratings. It is not what is honest or not true, but who can pull the most viewers. Television and radio stations have one thing in common; they live and die by ratings.

Because they are in such a rush to report it they don't care if it's lies and bull.
