i am looking for a form to document the weekly or monthly progess of counselling children. i am not 100% sure how do write up one and would be very thankful if someone could send me some links of some i could look at in order for me to get a better idea to create my own.

I use a weekly progress note to document treatment for adults with developmental disabilities. Contact me and I can arrange to send you a sample.
Basically I have the name, address, a space for medications, sce to write progress note of session, the purpose or goal of treatment, date and time of next session, and signature spaces.

Purpose or goal of treatment example: Sam receives individual therapy to relieve symptoms of depression, namely, the development of strategies to cope with anxiety and reduce excessive worrying.

In the progress note I outline strategies for the direct care provider to assist the person.

I know that many insurance folks want a note that shows how the patient is moving ahead in treatment and whether they felt the session was beneficial or helpful. When audited, I usually add this information since I provide a weekly note for the direct care provider and keep separate notes myself. The direct care provider usually does not want to know, nor do they need to now, everything that goes on in a session that the person reveals, especially if someone does not want them to know. Discretion is necessary.
