It is a name given to the moon when it is 'full' for the second time in one calender month.
If the moon was 'full' on the 2nd Sept, it would next be 'full' on the 29th Sept. The 'full' moon on the 29th is described as BLUE MOON.

a blue moon is the moon shining on a bright summers day,and the reflection of the suns rays bouncing back into space via the blue sea.thus making a blue moon.this is also why the sky is blue during the day.

when there's two, the second one is referred to as a "blue moon".

Hence the common phrase "once in a blue moon". (b/c it rarely ever happens)

It usually refers to something rare or that rarely happens. It refers to the rare occurance of 2 full moons in one month.
