You have to use the Forms feature of Word. Press F1 for Help and type in "form" for instructions.

What you want is a form. IN this way a file can be used for people to enter information which is then saved. The entered information does not 'affect' the layout of the page or the other pieces of information. Though this can be done in other programs, WORD has this feature built in.
You set up the form by inserting 'form fields' of certain lengths at the point where you want information input. You can even specify fields where certain types of information are input such as phone numbers. Word also allows fields for specific entry selections in the form of drop downs which you edit. (A drop down can be set to make the user select Mr. Mrs or Ms. as an example) The Help Index can allow you to learn more about each of these fields and how they are inserted, edited and used.
Once the form fields are in place, use the Protect Feature (on the Tools Menu) to keep the form from being changed, the fields from being adjusted or text from being deleted. Thus, the form becomes "permanent" in that information entered does not affect the layout of the rest of the page.
When a person uses the form, they can save it, and all information will save in the fields as they have typed it in.

There are several ways to do this, but the easiest ways are to either print and scan the document, or save it as a new file after entering the information AND convert it to a .pdf (Acrobat) file.
There are, of course, better ways to accomplish a permanent form than using Word. A database with the blanks in the form as fields is one way. However, if you need a unique signature on the form, scanning it is still he best way to go.

On the Word menu bar, click on Help.
Search for "forms" (without the " ").

Save it as a document template. Do a save as, then under file type in the drop-down menu select template.

try creating a "table" in word and putting the permenant information in a box, then have the next box over the one they would type into.

You can use adobe acrobat to turn it into a form. That would be the easiest.
