Central and Bill Dance charge a fee to join and yes that is normal for companies that hire extras. They are not 'agents' they are companies that do a service and that's that. ExtrasAccess is fairly new to me. It's an offshoot of ActorsAccess which is more for people looking for principal work and even though I've never used it, I trust it because of the people it comes from. Getting started is free, I'm sure there's a fee along the way somewhere, but I've never used it so I don't know where that comes in.
If you're in NY still go to Central Casting.
If you're anywhere else, you can try ExtrasAccess or you can also see if your state has a film alliance / commission / society or whatever your state calls theirs (some have more than one). Sometimes they post audition information for projects filming locally. Just do a web search for the name or your state and the word film and see what sites come up. Check them all for info!

You do not need an agent. You do not need to be in the union, though membership in the Screen Extras Guild will offer more opportunities. And technically you don't need headshots, although it will increase your chances if you have a nice image.
It depends where you live, but basically you can sign up with casting agencies who offer extra work. Some require a small fee, but cannot charge anything above that.

no... but its more work and thinking on your part.
1. find the auditions (news paper)
2. get head shots (very pricey)
3. make a resume
4. then audition for the part.
I live in Michigan and Ive gone to a few auditions for extras for the movies 8mile, 4 Brothers and The Island but I guess I wasnt good enough to stand around and take up space...lol cause i never got any parts...lol

When casting for a part directors usually call on agency's to fill the position. So it would help to have an agent find the parts for you.
