Under the work history section, it is asking for supervisor (name and title). Is the supervisor different from the employer? Also, in another box next to employer, it says title and under that it asks for my duties and pay and I understand that part, but when its asking for title, is it asking me or the employer?

Supervisor is the person to whom you answer and ask questions of, they oversee your (and maybe others)work. If that's the boss, owner, him/herself, put that name. Repeat "owner" where it asks title.
Where it's asking about your duties, that'd be your title doing those duties.

If you worked for a company the employer is the company name, the guy you worked for is the supervisor.

Its asking for your title according to HR what they called you, what were your duties, then pay in hours, week or yearly.

The employer is the company. The job title is yours. The supervisor is the name of the person you reported to - who gave you your assignments?

Yes. Your employer is the company you worked for and Supervisor is who you reported to.

Example:
Employer: Microsoft
Supervisor: Jane Doe
Title: Your title (since they know Jane Doe's and your employer's title)

Supervisor is the name of the person who was your direct boss. When they say "employer" they mean the name of the company. Sometimes its the same thing.
When they ask for title, they mean yours, what was your job title. It gives them a better idea of your experience.
