the cell phone can heat up. in cheap cell phones the heat can cause a short. i don't think there's any serious danger of injury - there isn't a terrible amount of power running through the wires - but a short will wreck your phone.
in older phones, charging while using will run the battery down much faster (NiMh batteries).
if you have a lithium-ion battery, then it should last for 1200 or so charging cycles. that is, if you charge it and run it down every day, it should last about 3-4 years. most experts agree that charging while using can add "cycles" and run down the battery a bit faster, shaving 10-20% off the shelf life of the phone.
in general, if it gets very hot (not warm.... very hot), then stop using it while charging.

most modern cell phones should be fine. as long as the wire is not damaged.
The only danger is from a power surge. But most houses have sheilding against this.
