how much does fate take its hand in this?

dont apply for it, they'll always promote their favourite ars'e lick over you. i got sick of asking for a decent rate of pay for all the graft i put in and got nothing in return. but thats life, the good 'uns get shat on.

Once, perhaps twice, per employer in a 2 year period.

As to whether it will ever happen or not depends on the reasons given why.

A non-exhaustive list of possible barriers to ponder are:
Are there not enough staff coming in at the lower levels to warrant your promotion?
Are you lacking in managerial skills?
Do they not have the money to fund the elevated responsibility?

Ultimately, I would suggest that you ask why your were passed over. (I'm assuming this recently happened, based upon the wording in your question)

I am stupid enough to never quit.

Instead it is better to try and find a solution. Maybe you need to change the way you work, transfer to a new department or even find a new job.

Maybe you need to look at some additional training or educatin. Sometimes someone with a qualification will get the job over someone who can actually do it.

Either way, all my career success has been the result of not giving up. If you give up you are sure to fail, if you try and you have a chance of success.

Promotion is not applied for it is the recognition that the company is giving knowing of your capabilities you have to handle more responsible position. You may apply for a senior position in other companies. In that case I would say apply apply till to succeed

How do you ask for a promotion? Do you just go up to the manager and say I have done really well at X for X amount of time and feel that I deserve a promotion.....What do you say?....... It's all rather confusing!

If you are being passed over for promotion and you do not know the answer why, then why not confront the person responsible for offering promotion and ask exactly what quialities they are expecting to promote someone.
