Dry and/or greasy skin due to poor oil composition of the skin. Thick greasy oils clog pores, lack of oil leads to dryness and irritation. You need thin oils to moisturize while dissolving and clearing gunk in your poors. Try fish oil or seafood. 2 tsp fish oil a day or 4 servings of seafood a week. Stick it out for at least 2 months; it will take a long time to replace all your oil. Any effect after 1-2 days is temporary or random; so even if it makes you break out a little at first, you haven't given it a full try yet.
In the short term you can wash and moisturize well, but that will only go so far. Plus excessive washing can be drying and excessive moisturizing can be clogging. Use a small amount of a light moisturizer, made with oil not jelly or grease. Often that means soybean oil or mineral oil. Mineral oil means mined from the ground. So soybean oil is usually better, though mineral oil won't cause too much harm. Clean with soap and water, not a harsh acne cleanser. Even then they only work so well. So you really need the seafood.
Antibiotics aren't really good for bacteria long term, they'll come back in force after. Short term they may help. After you get off them find some kefir with acidophilus listed first or 2nd to replace the friendly bacteria they destroyed. Studies show less illness when you have these bacteria, even outside the stomach in places such as the lungs and elsewhere. You want friendly bacteria to fill the void when the antibiotics stop, not harmful ones.
In the short term you might also try 100,000+ iu retinol vitamin A (a megadose, and too much for normal use) or one of the acne drugs that is similar to retinol vitamin A. It's some minor harm to your organs, but it helps against bacteria on your skin. At least it doesn't have the other long term drawbacks to your skin that antibiotics and many scrubs do.

Has he considered going to see a doctor about the acne? The doctor could prescribe him some ointment that might help. To keep my face and body free of acne I use Noxzema Exfoiliting Cream. It's 6 bucks for a big bottle of it and it lasts a while. Your skin feels great after using it. That might help! Good luck and I hope that he gets better!

Get the Murad Body Acne Cleanser and the Body Acne Spray. He needs to consume a lot of water to help clear it up too.

He should see a dermatologist for tests, it could be dermatitis to scabbies. I don't think acne makes you itch real bad. He's got some issues going on. Might be a allegic reaction to laundry detergent or the soaps and oils.
try using an oatmeal based soap and calimine lotion for the itch . He should stay away from oils , it is a magnet for dirt and will aid in clogging his pores .
Wear looser pants.
Wash bed sheets and clothing often since getting in them with oil will obviously attract dirt and bacteria to his sheets and pants, which can transfer onto him.

Try proactive. I bought it for my then-boyfriend, and it did wonders for his acne.
But if that doesn't work for your bf, he should go to a dematologist, because his condition sounds serious and abnormal. Some skin conditions are best treated with prescription ointment/medication. Prescription meds may cost $$$, but it typically requires few applications over a short period of time, but it will work over a longer period of time.

he should probably see a dermatologist. body acne is usually treated orally by taking pills prescribed by a doctor. as for his insecurities just tell him you don't care about his acne and that you like him just the way he is.

Sounds like he has psoriasis.
He needs to see a doctor for an effective steroid treatment. A general serach on the net should help you find some home remedies.

He might be allergic to the harsh chemicals in the soap, so you might have him use an exfoliating towel like Salux towels on Amazon, and no soap. Also, after showering moisturize his skin. Instead of using lotion, you might have him try using olive oil, jojoba oil or coconut oil.

try sulfur based soaps on his legs. it's available in all supermarkets and drugstores at reasonable price. it's known to kills the bacteria that prevents the acne from coming back.
