If you open your mouth and your gums are beet red and swollen and flaps in the wind and your teeth are swaying side to side, not to metion the malodrous odor from your mouth!

When the tooth is already wiggling in all directions, including vertical direction, the tooth has a poor prognosis of recovering from gum disease. It usually means that there is no longer any bone that holds the tooth within the socket. The bone has already resorb due to chronic infection caused by the microorganisms.
