Bunch of mixed up answers here. Oy. Based on my research and making calls you need to do it in this order:
1. Get a certified copy of your marriage certificate
2. Fill out form SS-5 for the Social Security office and take it there with the marriaeg certificate
3. Take certificate, and print-out of SS information to DMV and change your license
4. Take certficate to your employer with new DL to fill out your W-4
5. Change your car insurance information (ask for their policy, each place differs)
6. Take certificate and your DL/SS printout to bank
7. Get all your utilities changed based on their policy
8. Anything else not included such as health insurance, catalogs, e-mail, Triple A, etc.

I just got married this year. You will need to take your marriage license to the Social Security Administration office. Go online to their website and fill out the application prior to going, then it will only take you about 5-10 minutes once you get to talk to the person. Then go to the DMV with the license and be prepared to pay for a new card. If you work someplace, you will need to fill out a new W-4 and other forms your workplace may need. You will also need to go to your bank with the license and get your account changed. Also contact all of your credit card companies and request a name change and new card. For your car insurance you may also have to bring your license and get it changed, or you may need to bring your new husband if you want to be on the same one. The best thing to not overwelm yourself with, make a list of all of the cards in your wallet, and all the things you get in the mail, then go down the list and cross them off as you get them completed. Somebody let me in on this info when i got married and said the easiest way to do it was to go to the driver's license place first because it is really easy to change your last name and then you have a photo id with your new name on it right away. then take that to the social security office and get your card changed. After that everything is pretty easy because you'll have your two forms of id if anyone asks.

Upon getting married you need to go to your County Clerk's office...There u and ur fiance will have to fill out papers and sign your name...They will give you the copies that you need to get everything changed over...Once u get married your minister, priest, etc... will mail them in the original marriage certificate...once they receive it and seal it then they will give it to you with as many copies as you want...u will need this to change ur driver's license, social security card, bank information, anything that is legal with your old name on it...hope this helps!!! i got married Sept 15 and we received our certificate that week...it just depends on how long it takes the minister or whomever to turn it in and for them to seal it...but to answer ur question u do not do anything but file for a marriage license and the person doing the ceremony is in charge of turning it in and u will have ur new last name!!! good luck and best wishes

Legally, however you sign the marriage certificate is your new name. If you plan on usinghis last name, use it when you sign your marriage license. Then you'll just have to take that and your SS card to get a new driver's license.

In order to make it official you need to go to the Social Security office with your marriage license.
be sure to change your name with all other companies, too.
Cell Phone, Utilities, Car loan, insurance, etc.
Generally they will only need a copy of your marriage certificate.

from what I gather, you get any paperwork necessary when you go to get your license. In my state at least, you can change your name right on the license/certificate. I know this because I asked the people in charge, I forget their title and the exact specifics of it, but the gist is correct.

You have to have a copy of the marriage certificate ( I believe). You need to take it with you to change your driver's license, social securty card, credit cards, and anything else that you may have. It's a pain in the butt, so good luck!

It will change automatically when you get married. You just have to inform the Social Security office, state drivers license, banks of the name change.
