It sounds like you Believe there is a Heaven and a hell, because you choose not to go to either place. Although you don't give acknowledge him, you must believe there is a God because of the fact you state you have free will. Who gave you that free will?

The answer to your question is not multiple choice. It's not True or False. SIMPLY EITHER OR.

It's Gods will that is being fulfilled. He says "NONE SHOULD PERISH, NO NOT ONE", and he is not going to force his will upon us, so he gave us free will to choose between his TWO choices.

The form you need to fill out is found in the Bible, it's under the heading of LOST. Which is a sub part of the word HELL.
It appears you have already made your choice.

Good NEWS though, your free will gives you the right to change your heart and your choice.

Uh, unfortunately, the ultimate question has TWO, and ONLY two, options:

(1) enter into (accept) the covenant of the New Testament for the salvation of your eternal soul thru Jesus, Son of the one true God; or

(2) reject and deny the love and mercy of God.

It really IS that simple.

You can design your own form with you free will if that is what you want; but having free will not necessarily means that anything you want will happen.

Free will and control over the cosmos are two different things. If you don't choose one, you choose the other by default.

You have the free will to go to a predetermined place. Sorry, I meant to a place God knows you'll go to, but you have the free will to not go where you're fated to go.
