I don't know if you have to pay for each month or make one payment for the whole year. And how much would it cost?

I pay for it monthly, it is part of my cable bill (Toronto). I have no idea how much it is though.

On my cable system (Comcast), you pay for Center Ice during the first 4 months of the season. I think I paid $129 last year.

It depends if you want to pay for it monthly or by the year. I would choose monthly because if you don't like it you can stop paying for it.
