Short-sightedness and no post invasion plans whatsoever. Instead of reforming an existing army to pacify a volatile populace, they decided to turn professional soliders into unemployed terrorists with huge caches of weapons. On top of which, those brilliant visionaries decided to let Shi'tes to gain significant political power without any governing intellegence nor experience.
Therefore, we have the current situation which we sink $ 3 billion dollars a week without any prospect of things getting much better as time goes on. It's a wonder that US government has not gone broke by now.

You're absolutely right and it is a decision I protested, when it happened. This was one of Donald Rumsfelds very early, bad decisions. He didn't understand the mindset of the Iraqi people. He didn't understand the motivation of those under arms. He didn't realize that politics only existed at the top of the armies ranks, and the soldiers really just needed a job.
Rumsfeld often shot from the hip. He was egotistical, to a fault . Another example of his failing was to install Paul Bremer, a corporate type, but not a military man, to manage Iraq. What was that all about and where was George Bush during all this time?
The removal of leadership you ask about removing, that should have been Rumsfeld !

It is the same great wisdom with which every great war has been fought and won.
One cannot leave a military indoctrinated in the previous government's ruthless and corrupt beliefs and thus, who operated in support of tyranny to stand.
There is no great mystery here and no legitimate reason to ponder the decisions made or, the actions taken.

Rumsfailed, Cheney, Wolfowitz, and the rest of the PNAC mafia didn't want to share all the flowers and candy they expected to receive.
But seriously, when Boy George dressed up in his little fly boy outfit and minced around for the sailor boys declaring the war was over, he actually believed it. Yes folks, he REALLY IS that STUPID.

I heard not too long ago that the army wasn't disbanded but it just disintegrated when they expelled the bathists who were the military leaders.
Edit: I see where I got that. That's what Rumsfeld said about the 9th paragraph down. Quite an over simplification.

It is one thing to dissolve their military, but they had never disarmed them. Firing thousands with guns can have a blow back effect.

It was a mistake. We should have either killed them or put them in detention camps for the duration.
