It was all a hallucinatory dream/nightmare. Tim Robbins had been mortally wounded on the battle field and the entire movie was a dream as he lay dying. That's why he keeps "waking up" in different places. The very end shows him climbing Jacob's ladder to heaven (or somewhere). He never actually PHYSICALLY goes home -- he dies in Viet Nam (I think that's where he was).
The little boy who leads him up the ladder at the end is his dead son.

The battle for the soul of Jacob Singer... Jacob is dying in viet nam after being dosed with a designer hallucinogen clandestinely developed by the U.S. government to increase the effectiveness of their fighting forces. The chemist gives him this information. the entire movie is a sort of "flash forward" to what his life would be like if he had lived. Jacob s chiropractor (Danny Aiello) is an angel, notice the number of times Jacob asks him if anyone ever told him he looks like one. The other images are demons, the faceless figures on the subway, the man who s head bobs back and forth way too fast, etc... all demon s. the entire movie is just him seeing into the realm beyond, helped along by the drugs in his system,
On a side note, this was a fairly low budget movie, most of the special effects were done using low speed shots and speeding up the film, Danny Aiello is frequently shown backlit to give him a halo. BEAUTIFUL MOVIE, I saw this in the theatre when it came out and left Shaking, literally, my hands were shaking when I walked out....

I can't explain it, but I'll be watching this question because I would like to know too! I saw it super cheap at walmart a while ago, it looked interesting so I bought it and watched it. It was so weird! It might help to look on Imdb (Internet Movie Database) to find more info about it. They have discussion boards for every movie, I'll bet someone could help you out there :-)

the government sprayed an experimental drug (called the ladder) onto troops in vietnam. the drug made everyone revert back to an almost primal state and kill anything that moved. jacob (tim robbins, i believe) died during this. the movie was his refusal to accept/realize that he was dead, and in the end he finally figured out and accepted that he was dead, and his ascending the stairs with his dead son (macauly culkin) was his acceptance of his own death.
it was called "jacob's ladder" because it was the story of his "trip straight down the ladder" as the movie puts it.
