the whole of Spain is a pretty loose confederation. Only the Basque region wants to become an independent state.
In recent weeks/months...much has been said about the possibilities of a break-up of Belgium into several parts (Flanders, Wallonia..for instance).
Wales and Scotland could eventually break away from England. But i don't see that one happening anywhere soon.
Kosovo will become independent from Serbia
There has been a movement for independance in Corsica...but i guess that one is slowly dying out now.
Cyprus is independent by now. But there's a Turkey-dominated (military) part of the island.
Some Russian republics (chechen, dagestani,...) might dream of independance...but it doesn't look like it's going to happen.
Ofcourse, the stronger the EU gets, the less important the different nations will become, the more important the regions/city-states will become.

It is likely that the Celtic States (The Isle of Man, Scotland, North Ireland, Wales, Cornwall, and Brittany) would secede from England (and in the case of Brittany, France) and form either individual states or a union. Basque is also a major possability, as well as the Laplanders and Yugoslavians.

Basque, Sardinia, Corsica and Sami Laplanders all have separatist ideas about them.
Cyprus is an ongoing issue.
The the former Soviet republic of Georgia and the Chechen areas also have areas of unrest.
