You really don't know. There are a few blood tests and your doctor listens for a heartbeat but that leaves a lot of time for worry.
The best thing you can do is stay positive and notify your doctor of any changes or if you don't FEEL quite right.

Well the doctor or nurse will listen for the baby's heartbeat on the doppler, you will start to be able to feel the baby move and doctor will also measure your tummy or you can see it get bigger. All these things are a pretty good sign that everything is going well. Anything that does come up is best seen at the 18-20wk scan as any earlier and it might get missed. If all appears well at that scan you may not have another till you meet your bundle of joy!

I felt exactly the same, i worried every day about if he/she was ok so my partner brought me a heartbeat monitor/doppler off ebay and i honestly think that was the best thing i have ever been brought in my entire life, i know that it cant tell you alot but it was a weight off my mind just to hear that little heartbeat going. I personally think that making expectant mothers (especially first time one`s) wait up to 5 weeks between visits to the midwife disgracefull.
Anyway time will fly and when you get to about 20 weeks you`ll start to feel him/her wriggling around.

Honestly, there is no way to tell. You have to do your best to judge by how your body is changing (which occurs pretty slowly at this point) and how you feel...even that can be a difficult thing because some pregnancy symptoms, like nausea and vomitting, are going away at this point. Just pray, hope, and take care of your body the best you can. Take your prenatal vitamins, get lots of rest, and your baby will most likely be just fine :)

Because they would have been able to tell you at your 3 month scan, and they could have told with the blood work..
Unless you started bleeding, then you should be worried.
If you feel normal, then everything is probably fine... You wont more than likely be able to feel the baby yet. So if you can't feel him/her moving, don't worry

if you are healthy,and your doc is not concerned you are fine.
you do not need an ultrasound to tell you the baby is ok, this is why you go to the doc once a month, they check on the baby using a doppler.
do not worry.
