Their lungs aren't acclimated to the fresh air and it might kill them to breath it in.

When your nose gets used to a odor being around, it becomes desensitized and barely notices the odor. This is true of people who have less than fastidious hygiene, as well.
I knew a girl in school who smelled really bad. People openly made fun of her, teachers took her aside and spoke to her, and even the school nurse spoke to her. Nothing changed except she gained a huge amount of weight. I found out many years later that she was being abused by her brothers at home and hoped that by smelling badly, they would leave her alone.
Before you judge these smelly people to harshly, think about all the possibilities for why they smell bad. True there are some people who don't care how they smell, but there are many who either can't bathe or are afraid to bathe and some who smell no matter how much they bathe due to health problems.

they are so used or adapted to their environment or smell that they don't realize how bad they smell. On the other hand, people who's been out more, in the FRESH air, notice the bad odor.
