lol, i guess i can't put "I screwed up" on a formal essay. help me please.

I messed up, I made a mistake, I did the wrong thing, I made a bad decision, etc. Kinda depends on what was done wrong.

"Did poorly". "Made a bad job of" "Did not do my best" "Was careless with" "Had not prepared sufficiently for". Even "messed up" is better than "screwed up", so you were wise to ask the question.
