Do you have locks on your doors? Put a chain up high enough on the door where he/she can't reach. You could also try teaching the kid that he can't leave the house alone.

You must also talk to them and teach them that they can not go anywhere without supervision. I teach 4 year olds and they understand about dangerous situation, it takes aproximately 3000 time to explain something to a toddler for it to really sick in, so when having conversation with your grandchildren tell them what you expect of them. Also make sure the doors are locked and they cannot reach them, even with a chair.

put those child-proof things over your door knobs...
the ones you have to squeeze n turn a certain way to get out.
they even keep me in so i think they may work on a toddler haha.
