You will probably find that very few U.S. scholarships will be available to you - you need to look at Spanish-based scholarships. Perhaps your government has some for its citizens. You might also consider schools which may have exchange programs, wherein they send a student to Spain for a period of time and you come to their university. Usually, each pays at his or her home university.

One thing you ask about which makes me wonder. You mention starting the university, but you say you want to study medicine. Certainly here, medicine is a graduate degree (I think that is the case in Spain, too, although my only experience with the medical system there is with an emergency room in Madrid!). You need to finish a degree at the university before you can start medical school.

As to good schools, if you want to be in a cold city, the best places in New York are Columbia and New York University, in Boston, Harvard, Boston University, Boston College, and Tufts, and in Chicago, Northwestern University, the University of Chicago, and the University of Illinois at Chicago. You might also consider some of the schools in Minneapolis (the University of Minnesota), if you like really cold weather (what kind of a Spaniard are you?!!!).

As to learning English, there are so many language schools in Spain - I know that last year I was in Madrid and Pamplona, and we visited many different schools. Each of these teaches Spanish to foreigners, but also other languages (primarily English, in fact) to Spaniards.
