All i use is just an ordinary notebook that has A - Z (where pages are refillable). I'll write down the title, the summary or what my thoughts on that book. I also have this habit of jotting down phases that I find interesting in another journal and so forth. I have originally thought of looking for a website but then that would be too troublesome. Its so much easily accessible if i have my journals with me. Hope this helps.

I use a blank journal for this purpose. I bought it at a Borders book store. The cover has a picture of a bookshelf lined with lots of books. I write the title, author, and approximate date that I finished the book. I tried to include books that I read, as a child, but it's hard to remember them all. I included as many as I could and tried to remember how old I was, so I could at least give the year that I read it in.
I also keep a separate journal for listing books that I would like to read/own.

Why do you need a website to make a personal list. Buy a notebook and list your books or type them out and print them and put them in a notebook.

I bought a small notebook called, The Books I have enjoyed" with spaces for title, author, date and comments. I used to keep it on my desk at work for others to share their own thoughts.
I bought it at Barnes and Noble.
