Depends on what you mean by decisions the rat line runs from your fire-team leader up to George W. and then to whoever I suppose you have in mind. However, the major policy decisions are made by Bush and his close advisers.

That doesn't surprise me. In fact, I'd be surprised if he had the capacity to make a LOT of the decisions he gets credit for.

Responsible as in "should be held accountable" or responsible as in "making the decisions"? As Commander in Chief Bush should be accountable but he has delegated that responsibility to General Prevasious (forget how to spell/pronounce his name). I doubt if the Iraqis have much true say in the matter after all they don't bring much to the table to give them true leverage.
