This is very simple.
Firstly, get the freshest eggs you can. Put a deep saucepan on the heat, and half fill with water, and a splash of white wine vinegar. the white wine vinegar is acidic, and helps set the white of the egg.
bring to a simmer - but DO NOT BOIL - then get a whisk and in a clockwise (or anticlockwise manner) make a vortex - water swirling around the pan, with a dip in the middle - like you see when water swirls down a plug hole.
Break the egg and drop gently into the middle of the pan.
Leave to simmer for 6 minutes, and remove with a slotted spoon.

In the microwave - it is much easier to get a picture perfect poached egg from the microwave - simply place in a bowl 1tablespoon water and 1 egg perforate the yolk to prevent explosion - cover and microwave on high (100) for 45-60 seconds. The egg will be beautiful, this also works for salads if you scramble the egg then cook as above you can dice the "omelet" and top a salad or make quick egg salad. Alternately there are egg poachers or you could use the Pyrex sauce bowls(1/2 cup size) in a pan that reaches half way up the bowl bring water to a simmer cover and let steam for 3 minutes. I have a whole new suggestion compared to the other answers so I hope I get best answer!
Anyhoo...
- Get a small bowl and line it loosely with cling film (make sure you have plenty hanging over the edge too).
- Now boil some water in a sauce pan.
- Next, crack the egg(s) in the cling film lined bowl.
- Carefully tie the cling film in a knot through a kitchen spatula.
- Hold the spatula's handle (with the egg tied securely in the cling film on the other end of the spatula) into the boiling water.
- Lift up the spatula every now and then to check when the egg is cooked to your preference.
- Once the egg is cooked, un-wrap it from the cling film and serve!

I make mine in the microwave its easy, quick and not messy.
Microwave Poached Egg on Toast
1 slice bread, toasted
1 large egg
butter, to taste
salt, to taste
pepper, to taste
Place the toast on a plate, butter it, flatten the middle down with the back of a fork.
Crack and carefully pour the egg into the flattened middle of the toast, not allowing it to spill over into the plate.
Prick the yolk quickly and carefully with the tines of the fork and bake in the microwave on a turntable for about 1 minute on, until the white is cooked through or desired doneness is achieved.

when the water comes to the boil stir it so it forms a vortex, crack the egg into it, and turn heat down a bit
Lift out with ladle, put on kitchen towel to dry off, then add to toast.

Boil water in a pot, add a tablespoon of vinegar, crack the eggs into the pot and let them boil and set to the way you like them. Or buy an egg poacher pot.
