What borough? I live in Manhattan. Restaurant, show or what? Just take her through the many, many shops in Manhattan I know for fact she will find something unique that she has never seen before. Shopping is fun. Many restaurants of every ethnic cuisine. She will spot a restaurant and want to try it and she will love it. Just do a Manhattan at large tour, you'll find your way around and you will both have a night to remember.
It's too late for tickets to any shows anyway but shows are too confining and take up the whole night. You should do things and get back to your hotel or wherever before too late.
If you don't know how to hail a cab or are completely a stranger to Manhattan I suggest maybe not doing this right now. I've lived here 12 years and if you're not native or very familiar it can be overwhelming.
You'll be safe if you use common sense during daylight hours and early evening but don't run around late.

Call to make reservations first, but I suggest the following restaurants.
Gramercy Tavern, they have fantastic service and although the tasting menu is pricey, it is a four hour meal--no kidding.
Babbo, one of Mario Batalli's restaurants, they'll usually seat you at a table if you show up right at 5PM and wait at the bar.
The Tasting Room, it's a small restaurant, but they have a very friendly staff, great food and the best cocktails in Manhattan!
WD-50, they have a very eclectic menu with very interesting flavor pairings, you must be adventerous to try the food here, but the service and experience is amazing.

my husband took me to this restaurant called The Modern its actually located at the MOMA and is looking out in the garden, its really romantic and the food was great. take her out to a brodway show... wicked is great so is Mamma Mia
One if by land two if by sea too, we went there for valentines day this year... great place.
I saw someone suggest Russian Tea room --- waste of money, i am russian and the restaurant is not what it used to be.
If she likes stake the try The Palms.... HUUUGEEEE deserts there and really good food.

I think the most impressive thing would be jewelry. Nice jewelry. I love hearts, a heart encrusted pendant would be nice. If you don't have that much money, any kind of jewelry with initials would be nice - maybe a watch with an inscription on the back.
