First you go down to the Social Security Office (www.ssa.gov) for nearest near you.

"All documents must be either originals or copies certified by the issuing agency. We cannot accept photocopies or notarized copies of documents.
Documents for a name change: If you need to change your name on your Social Security card, you must show us a recently issued document as proof of your legal name change. Documents Social Security may accept to prove a legal name change include:
Marriage document;
Divorce decree;
Certificate of Naturalization showing a new name; or
Court order for a name change.
In addition to showing us a legal document proving your marriage, divorce or annulment, you must provide an identity document. That document must show your old name, as well as other identifying information or a recent photograph. (We can accept an expired document as evidence of your old name.)
(In this case, a birth certificate and driver's license)
Your card will be mailed to you, at least mine was.
DMV: Once you have your certified copy of marriage certifcate that usually comes in the mail with in 3-6 weeks, you can then go down to change your last name on driver's license. That costed me $27 to do.
Then go from any place you need to change your name: Work, Schooling, banking accounts etc. They will help assist you in format to change it in person.

Let me just tell you, that this is a long process and very tedious. If I would have known how much of a hassle it is, I don't know if I would have even changed my name.
First you need to get a copy of your marriage license that you submitted to the courthouse. This can take sometime, since they need to process it. You can not use the "souveneir" license that was given to you. It has to be a certified copy from the courthouse.
Your next step is to go to the Social Security office, and get a new Social Security card. This is where they will ask for your certified copy of your marriage license. This part is easy, but you then have to wait for the card to come in the mail.
Next stop, the DMV. They will also need a copy of your marriage license and your new social security card.
Once you have these 3 items, you can start changing your name on credit cards and your mail and such. Just call the different companies, and let them know you got married and you changed your name. Some will do it no problem, while others will want a copy of your marriage license.

Before going to the DMV you need to apply for a new Social Security card. You can either do this by going down to the nearest SS office or you can print off the application and mail in all the paperwork. Once you have your new SS card you can take that and your marriage certificate and go to the DMV for your name change there.

Weird i just preformed a wedding on the same date...are you her LOL
What you need to do is get a copy of your marriage license first...If you got married on the 15, it should be done by Friday.
You need to change your drivers license, your name on your SS card...If your name is on anything, even the phone bill, you need to call them and change your name. Just look at everything you have with the old name and just start making phone calls...You will need the marriage cert to change You drivers license and SS...
