One of you starts the talk with "where do you think this relationship is going?"

And it usually ends with you both promising to not see other people, which means you are boyfriend and girlfriend, or it ends with you fighting and being mad with each other.

I dated at first. Once we got to know each other and new we did not want to be apart. He asked me "do you want to make our friendship official", I said yes, and the rest was history. We have be bf and gf for 19 months, and happy. It really does not matter who makes the first move. If you both have the same feeling neither one of you will be turned down.

It happens naturally. As you hang around each other more, you slowly become more attached. Then people start asking if you're dating and you say yes. Then they ask if that's your gf/bf and you get comfortable saying yes. It's a natural cycle. Or you can go back to grade school style and ask with a note that says will you be my girlfriend, with boxes that say; yes, no, maybe. Either way works.

there is no specific way to become boyfriend and girlfriend. One of you when you feel its the right time ask the other out hopefully they say yes and typically the guy makes the first move

Gawd. The guy asks you to be his girlfriend. Just be patient. If he doesn't ask you after one two months of "dating," then be like, "Are you actually single? Because it seems like you don't want to have a girlfriend..." Don't be shy about comments like that. Kind of giggle while your saying it, or smile or something. But DON'T seem to anxious. And if he says something like 'Fine, will you be my girlfriend?" or whatever, then sigh, THEN answer yes. =)
