It is a sovereign city which operates both as a city and a state (nation). Before technology made it possible to maintain order over large groups of cities, city-states were common. Ancient Greece was made up of city-states.

The only true city-state currently in existence is Vatican City.

A city is an urban settlement with a particularly important status which differentiates it from a town.

City is primarily used to designate an urban settlement with a large population. However, city may also indicate a special administrative, legal, or historical status.

It's both. It's a city that serves as an individual. The only one I can think of is Vatican City.

On some forms, either City, or State will be enough information .

It could mean they want both, as, some people omit the State.

it usually refers to ancient times. large cities, such as Ninevah, & Babylon, were ruled by kings who ruled just the city, which enclosed fields, etc. as other answers say, modern examples are the Vatican, and Singapore. Singapore would more closely resemble the ancient examples.
