if you have above 12 employees you are required by law to provide restroom facilities but you do not have to have a canteen as such the restroom facilties have to have suffiecent chairs and tables for each employee and basic facilities for tea making etc anything above is at the employers discretion. it should also be away from the main place of work and completely seperate from the workplace itself , and also toilets and washbasins and in some case you may have to provide showers and hot water and heating etc

Some states, not all, have laws that you have to provide a place away from the work area for people to eat. I don't think any state mandates canteen facilities though.
