1. Using God's name as a swear word or meaningless exclamation.
2. Attaching God's name or approval to something that human beings came up with on their own.
3. Calling him "Lord" but not making him Lord (i.e., living out his commands).

The original context of taking the Lord's name in vain had to do with using the Lord's name in an oath to gain credibility but then not following through on the vow.
However, using the name of God like a curse word certainly shows disrespect to the eternal Creator.

I once heard someone say that taking the Lord's name in vain means to associate God with something he's not part of. That is, to attribute an action or idea to God that is not of him.

Using the name of the Lord, His Son Jesus, or The Virgin Mary (who is inseparable from the Holy Spirit) in any way that is insincere or as a curse.
The French phrase Sacre Bleu is a prime example of taking the name of the Holy Spirit in vain via cursing about the Holy Virgin.

to take something in vain is to use it with no result, or waste the attempt.
Preachers, praying over a little girl with heart, and lung disease and cancer to be healed by god/jesus would be a fine example of using the lords name in vain
because it produces no result

Well, something that is "in vain" is something that didn't have the desired outcome...
So, praying for a friend or relative to be cured of cancer, then they subsequently die from cancer would be praying in vain. Since you are, probably, praying in the Lord's name, that would be taking the Lord's name in vain.

As a Christian, I simply avoid swearing or rude talk altogether

If I was to call out your name and then say nothing at all, I would be using your name in vain.
When someone calls out our name there are times that we fail to hear.That is not so with God.If we so much as whisper His name He hears us.
