i understand that they want you to have an empty belly, but does it really take FOUR hours for the water to leave your stomach? Especially for a 2yr old?

How long does it take to digest water?

What about breastmilk? I understand breastmilk only takes 20 minutes to digest. Shouldn't that be okay then?

Do they just say 4hrs to be safe as a liability issue since I have seen some sites that say 2hrs is okay?

Well, lets put it this way. Do you really want to chance it that your 2 year old will regurgitate while under sedation and risk respiratory failure? Do you really think that 2 hours will make enough difference to a 2 year old that its worth risking their life over? I mean, seriously, when I went to have my wisdom teeth out, my appointment was at 8:30 am, and they said to not eat or drink after midnight, and it wasnt really that big of a deal. I don't mean to come off as rude or anything, but seriously, its a 2 hour difference, just dont give the kid anything.

I think it depends on whether or not you have food in your stomach. It varies. But either way, usually it's don't drink until after midnight, but I personally did eight hours before. They don't do this for a liability issue, they do this for the person getting surgery, if water or food is dranken too soon before surgery, then you can throw it up during surgery, and that can possibly be fatal. Be safe and don't eat or drink anything after midnight as the guideline, but most importantly, just follow your doctor's directions.
Edit: I just re-read your post, if it's a two year old, I believe the guidelines are different than nothing after midnight, I would just follow what your child's surgeon instructed you to do to prevent complications.
