I just took a reflexives quiz in Spanish and failed it tremendously, and I do not understand it.

The question is:
Make up an origional sentence.
Yo llamarse____

Do I put: Yo me llamarse or Yo se llamarse...

I am VERY confused!!

The above answers are quite complete, but I'm not sure they answer your question. I think your teacher also wrote a poorly formed question, but that's a different subject.

What he/she wants you to do is use "llamar(se)" in a sentence written in the first person. Basically, he/she wants you to write "Me llamo José" -- "I call myself Joe."

In reflexive verbs, the pronoun always agrees with the subject, which someone outlined well above me.

no. "yo llamarse" doesn't even make sense. because "llamarse" = to call one self. "yo llamarse" = i call oneself. an example of which you use that verb (llamarse) would be:
se llama = he/she/it calls his/her/it self
in many cases, that "se" at the end comes before the verb, so you won't need to write it again at the end. "llamarse" in this form is just the original form of the reflexsive verb. look at these examples....

me llamo = i call myself
te llamas = you call yourself
se llama = he/she/it calls his/her/it self
nos llamamos = we call ourselves
se llaman = they call themselves.

Yo me LLAMO _____

To use Llamarse - You would use it to say - He likes calling himself paco -> A el le gusta llamarse paco

Or, you can say its to call yourself.

You would put Yo ME Llamo

You have to conjugate the verb rugularly and put the pronoun before the conjuagted ver

so..
yo - i
me - Myself
llammo - I call

you have to put the reflexive pronoun. me, te, se, nos, os, se in front of the congugated verb
me ducho - i shower (myself)
me bano - i bathe (myself)
te duchas - you shower (yourself)
nos llamamos - we call ourselves
