Take him to the doctor.

Does he have chronic diarrhea or constipation? Does he seem fatigued? Could he be anemic?

The reason I ask is that an enlarged belly could signify a lack of proper nutrition stemming from Celiac's disease. This disease causes the villi in the intestines to kinda of flatten down and prevents them from "grabbing" the proteins and nutrients needed for proper health. The intestines cannot handle a specific protein called "gluten" and this can cause fatigue, anemia, diarrhea, constipation and can cause the belly to swell.

Take him to a doctor and see what he or she comes up with. If the doctor says irritable bowel syndrome or any other digestive disorder, ask him or her specifically about Celiac's. It's not curable, but can be kept greatly under control with the proper diet....which means no gluten. Gluten is commonly found in flour based products (bread, cake, etc.), but there are companies that make gluten-free bread and such.

The University of Kentucky is doing a study on a pill that can allow people with Celiac's to eat regular flour based products.

Good luck! Hope this helps!
