Introduce newcomers as they come in is the simplest way. Then let everyone gravitate to whomever they wish. If you know that some people may have similar interests but do not know each other, make sure they are brought together with a comment like, I know that you both like tennis or whatever. People tend to mix and find their comfort zones so it doesn't fall entirely on you as the host and hostess but whatever you can offer will help. You need to calm down, make sure everyone has their first drink, knows how to serve themselves after, and then let everyone including yourself mingle. Keep your bar fresh and your appetizers up and chat in between. Relax. It's only a party and people do tend to enjoy themselves when the atmosphere is comfortable and relaxed.

If you go back to what some would consider old-fashioned etiquette: A man is introduced TO a woman; a child is introduced TO an adult; an subordinate is introduce TO a superior; a younger person is introduce TO an elder person.
The form is on the order of "Jane, this is Bob Smith; Bob, Jane Doe."

I like many of the other answers. If you feel uncomfortable introducing people, though, you can have them wear name tags. Ice breaker games are also good. I know that sounds hokey, but sometimes it's a lot of fun if it goes well. Here's a list of some web sites with examples.
