It's an expression of amazement or bewilderment. It's a shortened form of "What in the Hell" (..."is going on here" or whatever). Which comes from the idea that you're comparing the scene in question to a scene in Hell.

not a lot....it just means"what's going on!?" or some similar question...but always with an exclamation mark.
i wonder where this phrase originated from, and why? interesting.

To me it is just a popular saying. It is a way of demanding what is going on. It is a bit crass.
