What do you mean by "upside down"? If you mean with the tines pointing downward, that's the proper way to hold a fork. If you mean the opposite, it's very common and I don't consider it to be slovenly or low-class. Some people shove food into their mouths and talk while chewing, and that is rude, but I don't see many do it, and I certainly haven't seen anyone on a food channel do it.

It depends upon where you live and what culture you are from. Europeans eat with the fork in the left hand and the tines down. Americans eat wit the fork in the right hand.
Chewing with your mouth open and shoveling food in is bad manners anywhere.
I would consider stabbing people with forks bad manners, too.

Being intolerant of others and their habits is a breach of etiquette, calling them on it is even worse. Cultural eating habits seem to be very misunderstood by people in general--and always have been. To connect this with a person's class or lack thereof, is not acceptable. The term bad table manners is subjective. How one holds their eating utensil should be of no concern to you. Unless it's being used on your person.
However, crude behavior and talking with one's mouth full is not acceptable, whether it be on TV or in other public venues--that is the only part of your etiquette question that is valid.

Fork tines down is considered the proper way to use a fork.
In America, though. more people use a fork with tines up.
The mouth open while eating is just plain bad manners and disgusting.

The upside down fork is traditionally the European way of holding a utensil. The issue with talking with food in your mouth is just plain bad manners.

My dad use to stab my hand with a fork everytime I chewed with my mouth open. I'm not so sure about the fork upside down. I always thought that looked kind of prissy actually.

Eating habits differ in different cultures.
Europeans eat with fork in left hand, tines down.
Americans eat with fork in right hand, tines up.
Chinese use chopsticks, & hold bowls up to mouth.
Each culture is appalled at the habits of the other.
Just do what you were raised to think is correct.

There is a difference between good manners and just being anal about what people do. Every situation requires a judgement call
