be formed for a female name? What sort of terms of endearment might be used between close friends? lovers? husband and wife?

Family name first. Like for my middle name, Keiko, my close relations will call me Kei-chan, but it's typical for close people to call your by first name-san.

Lovers will sometimes use the word, Anata, which is similar to "honey." However, Anata should not be used in the same way we use the word "you" in English.

To Japanese, it's very impersonal and you should always use their last name-san, if you are not close or first name-san, if you are closer.

Anata, Kare or Kanojo, (You, he, she) is typically used in circumstances where the person's identity who is being asked (i.e. general question, application forms, etc. ) is not known.

Your family name is always first, and for a close female Friend and sometimes male or whatever you can add chan to the end and shorten it, sometimes you can just use ai which means love.

Just shorten it and add chan to the end of the name. Such as changing Haruko to Haru/Haruchan.
Add chan to the end of the name.
Add chan to the end of the name.
Just use each other's names.
