Look at yourself in the mirror and say, "it's my fault." And each person other than yourself can say the same thing because its never ending.
We live and die by choices we make. And just because everybody in the world don't read this or don't care doesn't mean we don't have our part to play. Every body has a conscience and know right from wrong.
So, it's all our faults because everybody do wrong or make wrong choices whether intentional or not knowing!

Sometimes it's our own fault, sometimes it's someone elses constructive fault but mostly it's just happenchance.The trick for survival is to know which applies in every case and react appropriately.
If you think everything that goes wrong is your fault you will always be anxious and unhappy without need. If you always blame other people and never accept responsibility for what is your own fault then people will hold you in contempt and rightly so.

We each create our own reality and if something happens that we dont like while we are blaming others then we are not taking responsibility for our role in it. I look on things that 'every thing is right at the time' and if I dont like it then there is a lesson in it for me, or it will repeat itself in a similar way until I do get the message. Life is an adventure a bit boring at times but without the lows we cant appreciate the highs.

I am not aware of the details of all that is happening in your life, however, I can offer this insight:
I find the concepts of 'fault' and 'blame' to be completely useless, as they are forensic; something has already gone wrong.
Responsibility, on the other hand, is a much more useful concept. It allows you to be proactive and take control of your life.
There are victims in life, although in my experience, people tend to victimize themselves more than people are genuinely victimized by others.
Perhaps if you took responsibility for your actions beginning now, by making it a point to be present and aware, you may notice fewer things going wrong in your life.

Things go wrong for many reasons, some of our own making and some problems are foisted upon us by others. BUT it is not WHO is responsible that matters ... it is HOW we deal with the bad things that makes us grow emotionally. Just look at people like Jane Tomlinson who did so much good as an antidote to news of her terminal illness.

Sometimes one, sometimes the other. You shouldn't take the blame for everything that goes wrong in your life, but equally you shouldn't blame everyone else for everything that goes wrong either. The difficulty comes in knowing how to tell the difference.
One of my favourite sayings is "the problem is not that there are problems. The problem is expecting otherwise and thinking that having problems is a problem." - theodore rubin.
It's so true.

Our faults because a positive outlook on any situation will enable us to see the advantages of all situations.
And others faults, largely those we grow up with who by their criticisms, and attitudes, blind us to the prospects of positive thinking, until we are lucky enough to discover it for ourselves.

Only ours for allowing it. Our lives are in our own hands. I often say that if it wasn't for others my life would be perfect. But it isn't perfect simply because i allow others to use, abuse and take advantage. It is the easiest thing to blame another for the sh*t that happens. Not so. We allowed it in the first place. Whether by action or attitude. Its all down to us.

There is a saying, "man(woman) makes his/her own luck".
But generaly, problems are nothing to do with good or bad luck.Then again, it is near impossible to be carefull all the time to avoid problems. So, one in the other, things who go wrong can be seen as hazards.
