Ask her out in person, face to face.

Wanna write notes? Send poetry

Luck

Edit: The only good note would be:
"Need to see you in private about something important"

When you see her, ask her out

There is no such thing as a good note to ask someone out. Simply go up to the person and ask them out yourself. Don't write a note, you never know who else will read it. At least if someone declines you you can say you never asked them out, with a note there is evidence.

I am not trying to be mean, so please don't get offended: how old are you? If you are under 16, I think a note would be a good way to start. Try something like "What's up? Hey, have you ever been to [name of restaurant, or movie, or skating rink, or miniature golf place]? I'm thinking about checking it out this weekend. Do you want to go with me?"

If you are over 16, you need to work up the guts to talk to her in person first.

Do you like me? Check yes or no

Ok, you ask her parents to help you. First you put flowers in her bathtub. Then you make a trail from her front door to where you leave the note with hersheys kisses. THe note says "Now that I have showered you in roses, and kissed the ground you walk on will you go to ___________ with me""

Okay, one more. You buy her two boxes of cake mix- angel cake and devils cake. You write a note that asks her to the dance. If she says yes, she makes the angels cake and delivers it. If she says no, she makes the other one. Worst that happens is you eat chocolate cake instead of going on a date.

Don't write her a note!!
You should actually ask her out, in person! It's much more genuine that way

None, ask them out in person, or by phone. First find out what type things she enjoys doing. That way when you make a suggestion, it will be one that she will be interested in doing. You have a much better chance of getting the date that way.

A note? You don't sound old enough to be out of the "girls have cooties" stage.
