There are really no traditionally established Nunchaku forms dating back to any Kobujutsu lineages.

Some later practitioners created a form that is very good. There is a video on google filmed in Hokama Tetsuhiro Sensei's Dojo - Kenshikai, in Okinawa. The practitioner is wearing a cutoff sleeve t-shirt type top. It is not clear enough to learn from it.
