Misdemeanor or Felony warrant? If it's an old misdemeanor charge the warrant might not even exist anymore. If it's a felony, that's a different story. Even if he turns himself in in CA, MD might not want to bother bringing him back for a trial. So the warrant won't exist anymore but there will still be a record of an outstanding criminal charge against your fiance. And that means, no passport. Guess you will have to vacation here in the good old USA.

Tell him to turn himself in before the people in the passport office do!
No, really, take care of business at home before you take it out on the cruise. Deal with the warrant. Spend a couple days in jail. Clear up and restore his good name, then take the cruise when it's all over.
Tell him to be a real man and face up to what he's done, instead of running and hiding like a cowardly kid. He had plenty of courage to do the crime, now have him show he has even more courage to do the time.

He can apply but I don't think he'll get the passport until the warrant is cleared. As for being arrested? I would be consulting an attorney. Just in case.

He will most likely get arrested. Make him take care of the warrant. You don't want this hanging over your heads for the rest of your lives. Take care and good luck.

It depends on the warrant. He should just turn himself over and let the warrant be resolved so he will have no further issues.
