4-5 weeks old, bad diareaha, was a barn cat, when it was pooing its hole started bleeding

A kitten that young with diarrhea could dehydrate and die very quickly. The bleeding could be irritation from the diarrhea, or it could be something else entirely - he needs to be seen by a vet asap.

What are you feeding it? If you've been giving him cow's milk, don't. At this age, he needs a good-quality soft kitten food, maybe mixed with some kitten formula. A little bit of canned pumpkin is good too. Make sure he has plenty of fresh water.

a trip to the vet is a definite first step, but it could be something harmless, my cat started to bleed like this as a kitten. I took him to the vet and he couldn't find anything wrong, then one evening I caught the little b***er eating the corner of the carpet, which had hessian backing and of course on it's way out the hessian was irritating. I got rid of the carpet, it was very old and ratty, and the cat has been fine since.

I would take it to the vet as quick as possible. This could become dangerous for the kitten.

Worms everytime, worm it, change of food does this also, but it's more likly to be worms, check it's stool for any tape segments, or roundworm.
De-flea it also worms and fleas go hand in hand, they eat the fleas that cause the worms.
